Please read through the Information Sheet before picture day - there's lots of good info there.  Below that are front and back views of the Package Envelope (what you need to fill out and bring with you to your photo time slot (see the schedule here), and beneath that is the personalized plaque order form - note that Team Sponsors already receive a plaque, so this order form would be for coaches/team staff or others.
