We are looking for additional players for the U14 Division, to register follow th e registration steps below.

Two (2 ) copies of on-line Registration Form, both signed and dated on the first page, initialed on the second page. Fill out your forms online at www.eayso.org.
Check totaling $135 ( $95 Registration Fee + $40 Fundraiser Card Fee)
Copy of Birth Certificate or other government document if your child did NOT play with Region 94 last year. We do not accept any other form of Proof of Age
 
*Community Cards will be mailed to you once your child is pulled from the Wait List- You can sell them for $10 each and keep that money.

AYSO is an all volunteer organization.  Parents must be involved to make the program successful.  If you can help, go to www.eayso.org to fill out a volunteer form online. When completed print 3 copies to bring to registration. All volunteers must have a volunteer form on file. Open positions include coaches, referees, team parents board members and non-voting staff (see Region 94 tab for openings). Information will also be available at sign-ups.

 

 

 



Refund Information
Before season starts - 100% refund
Once player starts practice - 75% refund
Once uniform is issued - 50% refund
After 1st game - no refund
